Purpose:
SVBC Children's Ministry would like to provide the best possible church experience for your children. We aim to be a place where they can both encounter God and build a personal relationship with him. In doing so we recognize that all children are unique and learn best in different ways. As parents we would like to utilize your expertise on your child(ren) so we can create a positive church experience for them.

*Please note: this is a one time form we will ask you to complete and the information you provide will be shared only with those whom it directly pertains. Our Children's Ministry team will use your input to try and structure our programming in a way that meets the individual needs of your child and creates the best possible learning environment for them.
